For those without insurance or who do not wish to bill insurance the fee is $200. Session fees are due at time of service.
Dr. Dinehart is in-network with some Aetna and BCBS plans. Please check with your carrier to see if Dr. Dinehart is an in-network provider. Having insurance does not guarantee payment for your sessions. The patient is financially responsible for all sessions. Session fees are due at the time of service. He does NOT accept Medicaid (AHCCCS) or Medicare.
Dr. Dinehart does not offer EAP billing.
Please speak with your bishop about authorizing sessions to be paid for by the bishop. This must be arranged, with all paperwork completed, BEFORE your session in order to be billable to them.
Late cancels or no-shows are subject to a $200 fee if not cancelled at least 24 BUSINESS hours before your appointment. Please contact the office directly, at 480-988-5003, for all cancellations and reschedules.
Cash, Debit Cards, all major Credit Cards, Flexible Spending Accounts (FSA), and Health Savings Accounts (HSA). All credit cards must be present in order to be used for payment.
You have the right to receive a “Good Faith Estimate” estimating how much your care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
• You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes
related costs like medical tests, prescription drugs, equipment, and hospital fees.
• Make sure your health care provider gives you a Good Faith Estimate in writing at your first session. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service.
• You can also receive information regarding the No Surprises Act from your provider. This is for clients whose insurance is out of network with Dr. Dinehart.
• Make sure to save a copy or picture of your Good Faith Estimate.
